LIST OF DOCUMENTS REQUIRED ALONGWITH H. T. APPLICATION FORM

 

                                                 

1.                                Company’s prescribed Application form.

2.                                Site Sketch along with Key plan indicating Point of Supply.

3.                                Land Documents (If Private Land)

(a)     Conveyance / Sale Deed for acquired Land.

(b)     Receipt of Registration of Conveyance/ Sale Deed.

(c)      Copy of Index -2 (Land transfer document)

(d)     7/ 12 Utara, 8-A etc..

4.                                Allotment letter and Possession letter of GIDC if Land belongs to GIDC.

6.                                Short description of Process.

8.                                N.A. Approval of Competent Authority for the Land for industrial purpose.

9.                                If the land is on Rent or Lease then Rent/ Lease Deed shall be on Requisite stamp paper and registered under Registration Act for at least 05 (Five) tears.

10.                           Copy of the last month’s energy bill with payment receipt.

11.                           List of machineries to be installed HP Wise.

12.                           For partnership Firm  

(a)                    Copy of Registered Partnership Deed

(b)                    Power of attorney on Stamp paper in prescribed format for Authorized Signatory to sign the Application Forms, Agreement etc.

13.                           For Private Ltd / Ltd. Company

(a)                    Memorandum of Articles

(b)                    List Of Board of Directors (Latest) having details like Name, Permanent Address, Tel & Mobile Nos., PAN Nos etc. on Rs. 100/- stamp paper duly notarized.

(c)                    Form No. 32 for every change in Directors i.e. Appointment or Resignation of any Director as differed from Memorandum.

(d)                     Company Resolution giving Authority to Authorized Signatory to sign Document like Application form, Execution of Agreement, and Authority to Affix Common Seal of the Company on Agreement Execution.

14.                           Details of DG Set installed with capacity and comparison of energy units consumed of DG Set and DGVCL for last 12 months duly confirmed by concern EE (O&M).

15.                           Copy of registration as SSI unit, NSIC Unit or Ministry of Industries to establish Industry.

16.                           All documents submitted by you must be SELF ATTESTED.

17.                           You shall have to pay Registration charges at a rate of Rs. 10/- per KVA maximum up to Rs. 25000/- by cash or DD in favor of DGVCL payable at Surat.
